Personal Data Protection Act on Sharing of Patient Information – REMEDIC CLINICS

Our mission is to provide the highest standard of patient care incorporating a holistic approach to diagnosis and management of illness. This practice is committed to promoting high quality, patient-centred care with all health services working together in providing fully integrated care for the patient. Our practice offers a variety of patient-centred services that will be a mixture of panel-billed and private. We do not discriminate in the provision of excellent care and strive to provide the best care possible to our patients.

We endeavour to provide patients with the best and most current treatments, methods, materials, and equipment within our resource capabilities to ensure that the patients receive the high quality of service they have come to expect. We believe that patients come first. Teamwork is highly valued and encouraged within the practice to promote a harmonious and productive environment. We hold great importance in ethical and responsible behaviour as essential to maintaining the trust and loyalty of our patients.
What is Patient Information?
For each patient we have an individual patient health record (paper, electronic, or a combination of both) containing all clinical information held by our practice relating to that patient.
Patient information is anything that includes medical details, family information, name, address, email address, employment and other demographic data, past medical and social history, current health issues and future medical care, patient contact number, accounts details and any health information such as a medical or personal opinion about a person’s health, disability or health status.
How do we collect patient information?
We collect your information when we scan your identity cards, when we update your details verbally or over the phone, when you book an appointment online, or when a doctor updates your individual patient file during a consultation.
In other instances, this information may be collected from a third-party source. This may include relatives, or other health service providers. This will only be conducted if the patient has provided consent to the practice to collect their information from a third-party source; or, where it is not reasonable or practical to collect the information directly from the patient. This may include where the patient’s health is potentially at risk and their personal information is needed to provide them with emergency medical treatment.
How do we share your information with the other service providers in the REMEDIC CLINICS?
The clinics operates as an integrated collaborative network of service providers, all dedicated to providing the best patient-centred care. By sharing information across service providers, this allows for a seamless patient experience where we can ensure each individual and family receives access to affordable person-centred healthcare and support services in a timely and convenient manner.
There is no Patient Registration Form available in our clinics, at the moment patients who passed us their identity card, it means they are giving consent for us to use their information of de-identified data to contribute to continued improvement in healthcare and research.
And patients are providing consent for us to pass on their personal contact information to the other service and healthcare providers in the REMEDIC CLINICS, for the sole purpose of them contacting the patient in relation to specific services they believe patients may wish to be informed of.
This information is not extended any further than the service providers within the ZIPZIP Group of Companies, and you are free to opt out at any time by advising us of your wish to do so.
Accessing your information, complaints and obtaining further information
If an individual wish to:
- Complain to us about a breach of privacy; or
- Access his or her own information held by us; or
- Correct any information held by us concerning his or her own information; or
